Urgent: COVID-19 Impact Survey

RE: URGENT ACTION REQUESTED – We Want to Hear from You!
Exceptional Children’s Foundation (ECF) and the Department of Developmental Services (DDS) invite YOU to complete a self-advocate/family member survey to help understand how COVID-19 has impacted our community and changed your services.
DDS, regional centers, and service providers are working on new ways to safely support you. Completing this survey will help us identify what’s working, what’s not, and what is important to self-advocates and families.
This survey will be available online through Friday, December 18, 2020.
The four questions listed below specifically pertain to ECF, so please make sure you complete these.
They are question numbers 4, 9, 11 and 12:
- Question 4 is trying to understand who has helped you during the COVID-19 crisis.
- Question 9 is asking about the types of supports and services you have received from ECF.
- Question 11 is asking if you feel you have received enough support. And, if not, why?
- Question 12 is asking if you are satisfied with ECF’s services during the COVID-19 crisis.
Once you click on the survey link above or here, you have the option in the top right corner of the page to select alternative languages (Spanish, Arabic, Armenian, Chinese (Simplified), Chinese (Traditional), Korean, Persian, Tagalog or Vietnamese) from a drop-down menu where it says “English.”
Each person (computer or smartphone) can only complete the survey once, so if you have more than one family member receiving regional center services, your response should reflect your family’s overall experience(s).
